Our Case Managers are licensed nurses and social workers. They provide important services like skilled assessments and helping patients find cost-effective solutions. Their wide range of resources makes them a valuable team member. Case management services is used to locate, coordinate, and monitor necessary appropriate services. It may be used to encourage the use of cost-effective medical care by referrals to appropriate providers and to discourage over utilization of costly services. Case management may also serve to provide necessary coordination of non-medical services such as vocational rehabilitation, education, employment, when the services provided enable the individual to function at the highest level.
The mission of our agency is to provides services which assist an individual in gaining access to needed medical, social, educational, and/or other services Our Case Managers, which include Nurses and Social Workers, provide these services through professional assessments and promotion of quality and cost-effective interventions to ensure optimal client wellness and autonomy.
Through Adult Care Management, a Care Manager partners with adults to get the care they want to improve their health and supports. Care Manager facilitates connections between health care and service providers to support individuals who are enrolled in Medicaid and have qualifying health conditions to remain in their homes and communities. When you choose to enroll in Adult Care Management, you gain a Care Manager, the key point of contact in helping you manage your needs.
The Adult Care Management program serves adults age 18 and up in Nassau, Suffolk counties and the 5 boroughs in NYC who are enrolled in Medicaid, Medicaid Managed Care, or Medicare and have two or more chronic health conditions (physical, mental health, or substance use diagnosis) .
These services provide care coordination to youth and adults with chronic medical conditions to get the care they need to improve their health and supports.

You can contact us any time to find out if you are eligible to enroll and to complete a self-referral. You can also be referred by Managed Care Plans, doctors, specialists, social services, counselors, or other service providers, though you cannot be enrolled without your consent.
