The Adult Care Management program provides essential case management services for adults aged 18 and older in Nassau, Suffolk counties, and the 5 boroughs of NYC. This program is designed for individuals enrolled in Medicaid, Medicaid Managed Care, or Medicare who are living with two or more chronic health conditions, including physical, mental health, or substance use diagnoses. Our focus on healthcare coordination ensures that participants receive the comprehensive support they need.

Additionally, you can be referred by Managed Care Plans, doctors, specialists, social services, counselors, or other service providers for case management; however, you cannot be enrolled without your consent. Our team is dedicated to ensuring effective healthcare coordination throughout the process.
