This program is designed for individuals enrolled in Medicaid, Medicaid Managed Care, or Medicare who are living with two or more chronic health conditions, including physical, mental health, or substance use diagnoses. Our focus on healthcare coordination ensures that participants receive the comprehensive support they need.
