Adult Care Management

Through adult care management, a dedicated Case Manager facilitates comprehensive care that incorporates the desire outcomes of the patient. The case management approach facilitates communication between all care providers, reducing gaps in care and ensuring individuals receive the appropriate level of care for their conditions.

Support Services

Our adult care management program provides the focus on linking individuals to community-based support services to strengthen your support network. Our Case Managers will assist in identifying and connecting you with the appropriate resources and services to address your medical and social needs.

Prevention and Geographic Area Service

The C YES CARE management program connects clients to appropriate services that may be available to them in their geographic area. Healthcare coordination may include the organization of non-medical services, such as vocational rehabilitation and education, to empower individuals to function at their highest level.

Care Coordination Services

These services offer healthcare coordination support for both acute and post-acute conditions. Our Case Managers will coordinate the necessary care to help you navigate through the healthcare system.

CPR Class

CPR-CLASS: American Heart Association BLS Basic Life Support (BLS) for Healthcare Professionals. BLS is designed to help healthcare providers promptly recognize several life-threatening emergencies, give high-quality CPR, use an AED, and provide early use of an AED. This course is beneficial for healthcare coordination and is particularly beneficial for professionals in case management.
